Our Care Navigators can assist you with your request to register with the Practice. We hope you will soon get to know how we work! It can take up to 2-3 weeks to be fully registered on the system so we always advise that you have enough medication to see you through this registration period. These can be obtained from your previous practice. 

New patient health checks are advised for patients with multiple diagnoses and medications so we can ensure that care is continued appropriately and this allows us to catch up with any problems before your records arrive, and also gives you the chance of a good check-up.

You can complete a registration form before coming to the surgery however you will also have to complete a GMS1 new patient form, which you can download below or collect from reception upon registration request.  When you come to the practice to register we ask you to have both forms completed along with 2 forms of ID, one photo identification and one with proof of address on.

We ask that each adult and any children over the age of 12 are present when registering at the practice. Children under 12 years old can be registered by the parents without them being present.  We acknowledge and accept that this may not always be possible but we are here to help and your registration will still be welcome.
